GPNs are encouraged to adopt a holistic approach to the care of individuals, for example:
• Offer, provide and signpost up-to-date advice and information on health and wellbeing.
• Listen to and support individuals, communicate concerns, refer (with consent) and deploy actions to achieve improvement.
• Contribute to the development of community resources to support improvement, targeting provision of specific programmes, responsive to local need.
